If you are a new patient to our office, please visit our Patient Forms section to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

At the end of the day we all have to understand that it is the patients responsibility to understand and know their insurance since this is a contract they have agreed upon along with their employer and carrier. Insurance is only a way to help pay for dental care and every little bit helps. We always are willing to help you navigate through your insurance but we are not responsible for the details and limitations of your plan. Our only responsibility is to prescribe what's best for you.
